Intake Form

Please print out the intake form and bring it with you to your first appointment. If you can email it to us (hello@integrateandthriveohio.com) before your appointment, that would be great! This way Shelby may get the chance to look over it beforehand to save some time in the appointment.

If you prefer, you can show up early for your appointment and fill one that is already printed out in the waiting room.
